PROCEDURES FOR NEW BUSINESS

STEP 1

Applicant secures and fills up application form at office (or download thru Internet) and submits the same at CMO -BPLD.

Requirements:

1 DTI Trade of SEC Registration or CDA Registration

2 Barangay Business Clearance

3 Community Tax Certificate

The documents will be forwarded to joint Inspectorate team to ensure that the establishment is compliant with the laws and regulations.

Joint Inspection Team:

  • City Assessors – for assessment of properties
  • CPDO – for Zoning Clearance
  • BPLD – for capitalization, determination of correct line of business and other details
  • CEO – for Building and Occupancy Clearance
  • CENTRO – for ENR clearance
  • CHO – for Sanitary Permit to Operate
  • BFP – for Fire Safety Inspection Certificate

During inspection, if there is a deficiency, the applicant is notified or informed of the deficiency.

After inspection, a staff from CMO-BPLD routes the documents to secure the necessary clearances, then forwards the same to another CMO-BPLD staff for encoding and preparation of Billing Statement, then the applicant is not notified.

STEP 2

Applicant retrieves the documents from CMO-BPLD and pays billing at the City Treasurers’ Office.

STEP 3

Applicant proceeds to BFP for payment of FSIC.

STEP 4

Applicant submits all documents to CMO-BPLD for sorting, checking, printing, approval and releasing of business permit.

Note:

While the CMO-BPLD staff are processing the documents, the applicant should secure SSS, PhilHealth and Pag-IBIG Clearances, BIR 0605 Annual registration and other requirements depending on the type of business. (Please refer to list of requirements.)

FDA – Drug Stores, Bakeries, other manufacturers that may be required of such clearance

CHED/DepEd Registration– Schools and educational Institutions

Customs Clearance – Customs Broker

Dept. of Energy Clearance – LPG dealers and retailers

DOTC – Courier and Messengerial services

NTC Clearance – Telecommunication related services

Pest Control License from Fertilizer & Pesticides Authority

Dept. of Agriculture – insecticide & pest control services

PCAB – General engineering & specific contractors

PSUCIA, license to operate – Security agencies

Trained Life Guards’ Certificate – Swimming pools and other resorts

Optical Media Board – Video rental services

NFA Permit – For rice dealers & retailers

TESDA Clearance – Vocational & Technical Schools.

 

 PROCEDURES FOR RENEWAL of BUSINESS

STEP 1

Applicant secures computer generated Application and Local Imposition Assessment Sheet (ALIAS) and Sworn Statement of Gross Sales Form (they can secure this as early as first week of December prior to renewal period.)

Requirements:

1 Barangay Business Clearance

2 Previous Business Permit

STEP 2

Applicant proceeds to CTO for approval of Gross  Sales, RPT Clearance, Assessment and Billing and Payment

STEP 3

               Applicant proceeds to BFP for Payment of FSIC.

STEP 4

Applicant submits documents to BPLD for shorting, checking, printing, approval and releasing of business     permit.

Note:
